Risk Assessment
The risk of reoffending is based on the substance use evaluation, a review of relevant records, and results from screening tools such as the Alcohol Use Disorders Identification Test (AUDIT). Two important factors in determining this risk are whether the individual has maintained a meaningful period of sobriety and whether they have a support system in place to help prevent relapse.

- Evaluations are conducted by telehealth. The practice works with clients in Massachusetts, New Hampshire, and Ohio, consistent with licensing and telehealth arrangements—confirm details when you schedule.
- How are evaluations conducted?
- Sessions are remote via telehealth. You need a stable internet connection (WiFi recommended). After you submit a request, we follow up to schedule and explain next steps.
- What does “unbiased evaluation” mean?
- The assessment is independent and may or may not support your license reinstatement or court goals. Findings are based on the interview, records as applicable, and standardized tools such as the AUDIT.
- What is the fee and when is it due?
- The evaluation fee is $500. Payment is arranged after you agree to an appointment time; appointments are not held until payment is received. You will receive instructions (including a payment link by text when applicable) after scheduling.
- What information will I need to provide?
- The request form asks for contact details, address, date of birth, convicting court, and your electronic signature acknowledging the payment terms. We may request additional records or information as part of the evaluation process.
